THE PROBLEM:The prevalence of diabetes in adults in India has assumed staggering proportions, with an expected increase of around 170% in developing countries compared to only around 45% in developed countries. THE CAUSES: Diabetes is a silent killer causing gradual and extensive damage to the small filtering units of the kidney (glomeruli) leading to excretion of large amounts of albumin (Albuminuria) in the urine.The loss of protein leads to reversal of albumin/ globulin ratio leading to retention of water by the body and oedema. The excess protein clogs the fine filtering units which eventually stop functioning and die. This eventually leads to renal failure in late stages of the disease. Simultaneously, the process also damages blood vessels and nerves that innervate the kidneys and can prompt an increase in urinary infections in the diabetic patient. Interestingly, only 20-30% of diabetics will develop diabetic nephropathy for reasons not sufficiently understood, but possibly attributed to family history, smoking, obesity, hypertension and hyperglycaemia (uncontrolled blood sugar levels) in the early stages of diabetes.

THE SYMPTOMS:The earliest symptom of diabetic nephropathy is a mild swelling of the feet specially around the ankle, which may aggravate and the whole lower limb below the knee may be swollen.Undetected and suspected the person may devolop anemia , shortness of breath, swelling of face ,weight gain and bloating of abdomen due to collection of water in the abdomen. The kidney if not rescued at this stage,can not recover and the kidney shrinks in size and chronic renal failure ensues.Chronic renal failure requires continuous dialysis or Kidney transplantation both of which are very expensive and fraught with complications and early death.

THE CURRENT MANAGEMENT: Early screening and corrective therapeutic strategies to avoid deterioration of symptoms like hypertension, elevated blood sugar levels, weight control etc. could help in management and successful treatment of diabetic nephropathy and avert serious renal damage and subsequent requirement of dialysis or kidney transplantation. Tight control of blood sugar and medication to reduce loss of protein in urine can slow down and postpone chronic renal failure.
